for the 2023-2024 school year

My family has been offered a seat(s) with Merit Academy for the 2023-24 school year.

I understand Merit Academy is a choice-in school and I have 72 hours from the time of offer to accept or decline.  If there is no response within that timeframe, that seat will be given to the next student on the waitlist.

If I no longer wish to enroll my students with Merit Academy, I understand I can complete the form below and select "Decline Offer" . These seats will then be provided to other students. My students will no longer be on the list for Merit Academy seating for the 2023-24 school year.

Submission of this completed DECLINE-form by a parent or guardian indicates the student(s) removal from Merit Academy's seating and Merit Academy's waitlist. I understand these seats will be provided to other students. My students will no longer be on the list for Merit Academy seating or enrollment.
